I’ve seen the heartbreak of patients who spent their life savings on expensive injections, only to find out they were treated in a rented back room with no medical oversight.     1. The “Miracle” Red Flag: Truth in Advertising

    The first thing that sets me off? Clinics that promise results. In orthopedics, there is no such thing as a 100% success rate. If a clinic tells you that you will be “pain-free in three days” or uses words like “guaranteed” or “miracle,” walk out. Ethical medical providers speak in probabilities, not promises. They focus on functional improvement and recovery goals, not marketing fluff.

    When you are interviewing a clinic, ask yourself: Does this feel like a doctor’s office, or does it feel like a timeshare presentation? A professional clinic will give you a realistic assessment of what the procedure can (and cannot) do for your specific condition.

    2. Who Is Actually Touching Your Body?

    One of my biggest pet peeves is the “phantom provider.” Many clinics use the name of a famous, highly credentialed surgeon on their website, but you may never see that person. You might be treated by a mid-level provider, a nurse, or even someone whose medical background is entirely unrelated to orthopedics or sports medicine.

    Questions to ask about the provider:

    • Who is the physician performing the procedure?
    • Is this physician board-certified in orthopedic surgery, physical medicine and rehabilitation (PM&R), or interventional pain management?
    • How many of these specific procedures has this person performed in the last 12 months?
    • Will I have a one-on-one consultation with the provider, or a sales representative?

    3. The Importance of Proper Screening and Medical History

    A reputable clinic doesn’t treat every patient who walks through the door with cash in hand. If they haven’t asked for your imaging (MRI, X-rays) or your full medical history before telling you that you are a “great candidate,” they are not practicing medicine—they are selling products.

    Screening and aftercare are the pillars of safety. If your condition is too advanced—for example, bone-on-bone arthritis where the joint space is completely obliterated—stem cells may not be the right solution. An honest doctor will tell you that. A predatory clinic will tell you anything to get you to sign the consent form.

    4. Clinical Protocols: Where is it happening?

    Stem cell procedures carry a risk of infection. This is why sterile environments are non-negotiable. I once turned away a clinic because they were performing injections in a space that looked more like a kitchen than an operating room.

  • “Can I speak to someone about the follow-up plan?” A procedure is just the beginning. Who manages your physical therapy? Who tracks your improvement at the 3, 6, and 12-month marks?
  • “What exactly happens if I have a complication or an infection on a Saturday night?” I want to know who answers the phone when the office is closed. If there is no after-hours line, there is no medical accountability.
  • “What is the source of the biological material?” Ensure you understand the distinction between autologous (your own cells) and donor-derived products, and ensure all regulatory standards are met.
  • “What are the specific risks of this procedure in relation to my medical history?” They should be able to walk you through the potential for flare-ups, infection, and treatment failure.
  • 6. Why Transparent Communication Matters

    Transparent communication is the hallmark of a doctor you can trust. They should be willing to explain the evidence, the unknowns, and the risks. If a provider deflects when you ask for clinical data or tries to rush you into a decision, they are prioritizing their revenue over your health.

    Ask for a breakdown of the costs. Is the price for the procedure only, or does it include the follow-up visits? Do they require you to bundle the procedure with expensive, unnecessary supplements or “wellness packages”? These are common upsell tactics that have no place in evidence-based medicine.

    Final Thoughts: Take Your Time

    You only have one set of joints. There is no rush to undergo a stem cell procedure. The science is evolving, and it is exciting, but it is not a substitute for a thorough diagnosis and a comprehensive treatment plan that likely involves physical therapy, lifestyle changes, and proper imaging review.

    When you leave the clinic, you should feel educated, not pressured. If you feel like you are being sold a dream, go home and think about it for a week. A good provider will still be there when you return. A bad one will be on to the next unsuspecting patient.

    Remember: If it sounds too good to be true, it is. Stay skeptical, keep your medical records close, and always demand the highest standard of care.
